I confirm that I am vaccinated against or have natural immunity to the following preventable diseases:

MMR (Measles, Mumps, Rubella)
Varicella (Chicken Pox)
Pertussis (Whooping Cough)
Influenza A (annual immunity)
TB (Tuberculosis) - completion of TB Risk Form initially
I can provide evidence upon enrolment to demonstrate up-to-date immunisations or natural immunity in line with the Australian Government and Queensland Health.
I acknowledge the robust nature of the NextGen Medics Program and understand that participating in this program will require travel and work observations over lengthy, daily periods. If there are any current or past health or other conditions or concerns, which will hinder my ability to participate fully in the program at all times, including physical exercise, working in semi-confined spaces, travelling long distances or elsewise, these will be raised by me and discussed with the Program Coordinator, at interview stage.
I can commit to the whole NextGen Medics program and 100% attendance over the block of the specified dates. Full attendance is required. There is no opportunity for flexibility. Please ensure you are available during the program date/s selected, including the Graduation Ceremony date BEFORE submitting your application.
I acknowledge and agree that should I be accepted into the NextGen Medics program, I am recognised as a NextGen Medics Alumni and Heart of Australia Ambassador. Within this role, I am to uphold the values of the Heart of Australia organisation and positively promote the program during its duration and thereafter when promoting the program.
I acknowledge that if I am successful at being offered a place with the NextGen Medics program, I will be asked to pay a non-refundable $200 program fee. This fee will be redeemable against the purchase of a Heart of Australia Gala dinner ticket. The Gala will be held on Saturday the 24th of October in 2026 at the Royal International Convention Centre in Brisbane.
I confirm that all the information provided is true and correct to the best of my knowledge and acknowledge that any false or misleading information may result in my application being withdrawn.
If your NextGen Medics application is shortlisted, you will be required to sign a Confidentiality Agreement and submit clear copies of your current Student ID, current Driver's Licence/Passport, and Immunisation Statement History.
